What Dust Mites Are — and Are Not
House dust mites are microscopic arachnids — closely related to spiders — that are too small to be seen with the naked eye. They feed on shed human skin scales and thrive in warm, humid environments. The allergens that cause symptoms come not from the mites themselves biting people (they do not bite) but from the proteins found in their fecal pellets and body fragments. These particles are lightweight and easily become airborne with household activities.
Dust mites are distinct from bed bugs: bed bugs are visible insects that do bite, while dust mites are invisible arachnids that cause allergy through inhaled proteins. The two are often confused by patients but require completely different responses.
Where Dust Mites Live
Dust mites concentrate wherever people shed skin and where humidity supports their survival. The primary habitats in a typical home include:
- Mattresses and pillows: The most significant source due to warmth, humidity, and proximity to sleeping humans throughout the night
- Upholstered furniture: Sofas, armchairs, and cushions that receive regular use
- Carpets and rugs: Particularly wall-to-wall carpeting, which is difficult to clean thoroughly
- Stuffed toys and soft bedding items
- Curtains and fabric window coverings
Hard floors, wood furniture, and leather surfaces are substantially less hospitable to mites than soft, fabric-covered surfaces.
Symptoms
Dust mite allergy produces classic allergic rhinitis symptoms that are typically present year-round rather than only during pollen seasons: nasal congestion, runny nose, sneezing, and itching of the nose, eyes, and throat. Ocular symptoms such as redness, watering, and itching are common. Symptoms are often worse in the morning after a night of sleeping in close proximity to mite-populated bedding, or after making a bed or vacuuming. Connection to Asthma and Eczema
Dust mite sensitization is one of the most commonly identified triggers in allergic asthma. Inhaled mite allergens can provoke bronchospasm, increased airway secretions, coughing, and wheezing in people with comorbid asthma. Managing dust mite exposure is therefore often a component of asthma management plans, not just rhinitis management.
Dust mite sensitization is also associated with atopic dermatitis (eczema) flares in some individuals. The relationship is complex and varies by person — some patients with eczema notice worsening after high dust mite exposure, particularly in bedroom environments. Diagnosis
Dust mite allergy is confirmed by allergy testing: either a skin prick test or an allergen-specific IgE blood test. A positive result indicates sensitization to dust mite proteins. Sensitization must be interpreted alongside clinical history — a positive test in someone with no relevant symptoms requires careful interpretation. Bedroom Exposure Reduction
The bedroom is the highest-priority area for intervention because most people spend 7 to 9 hours there each night in close proximity to mattress and pillow allergens. The most important measures are:
- Allergen-proof encasements: Mattress and pillow encasements with tightly woven fabric or impermeable membranes are the single most effective intervention. They prevent mite allergen from reaching the sleeping surface and block accumulation inside pillows and mattresses.
- Washing bedding regularly: Washing sheets, pillowcases, and blankets regularly in water that kills mites and removes allergen is important. Follow the care instructions for your specific bedding; your allergist or the clinical practice guidelines from AAAAI or ACAAI can provide guidance on effective washing conditions. Removing washable items from the bedroom reduces overall allergen load.
- Reducing soft furnishings in the bedroom: Replacing wall-to-wall carpet with hard flooring, minimizing upholstered furniture, and removing stuffed toys (or washing them regularly) reduces habitat.
Humidity Management
Dust mites require ambient humidity to survive — they absorb moisture from the air rather than drinking water. Maintaining indoor relative humidity below approximately 50% (ideally between 30–50%) significantly reduces mite survival and reproduction. Air conditioners and dehumidifiers are effective tools for maintaining lower humidity. In naturally dry climates, dust mite populations are generally lower.
Vacuuming and Cleaning
Vacuuming is beneficial for reducing the accumulation of allergen-containing particles on floors and upholstery, but it has an important limitation: vacuuming temporarily stirs up settled allergen particles into the air, potentially worsening symptoms in the short term. HEPA-filtered vacuums are preferred because they trap fine particles rather than recirculating them. Some people with dust mite allergy choose to wear a mask during vacuuming. After vacuuming, allowing time for the air to clear before re-entering the room is reasonable. Damp mopping of hard floors is more effective than dry sweeping for reducing airborne particles.
HEPA Filtration Limitations
HEPA air purifiers can reduce airborne allergen particles, but they have a specific limitation for dust mite allergy: the primary dust mite allergen particles are heavy enough to settle quickly onto surfaces rather than remaining continuously airborne. Unlike cat dander (which is very lightweight and stays airborne for extended periods), settled dust mite allergen is not efficiently captured by air cleaners running between cleaning events. Treatment Options
Environmental control reduces exposure but does not eliminate the allergic sensitivity. Pharmacological treatment options include:
- Intranasal corticosteroid sprays (now available OTC): the most effective pharmacotherapy for chronic nasal symptoms from dust mite allergy
- Oral second-generation antihistamines: effective for sneezing, itching, and runny nose
- Allergen immunotherapy: subcutaneous allergy shots and an FDA-approved sublingual dust mite tablet (Odactra) can modify the underlying immune response for appropriate patients
