What Natural Rubber Latex Is
Natural rubber latex is a milky sap produced by the Hevea brasiliensis tree, which is processed into a wide range of products including medical gloves, surgical equipment, balloons, rubber bands, and many household items. The proteins in natural rubber latex can trigger immune responses in sensitized individuals. Latex allergy is distinct from reactions to synthetic latex materials — products made from nitrile, neoprene, or polyvinyl chloride — which do not contain the same proteins. Types of Latex Reactions
Three types of reactions are associated with latex exposure, and they differ in mechanism and severity. IgE-mediated immediate hypersensitivity is a true allergic reaction and the most clinically serious type. It can cause hives, angioedema, rhinitis, conjunctivitis, asthma symptoms, or anaphylaxis, typically within minutes of contact. Delayed contact dermatitis (Type IV hypersensitivity) is a T-cell-mediated reaction that causes skin redness, itching, and rash, typically appearing 24 to 72 hours after contact — it is often caused by the chemical additives used in latex manufacturing rather than the latex proteins themselves, and may occur without IgE-mediated sensitization. Irritant contact dermatitis is the most common reaction and is not immune-mediated at all — it results from mechanical irritation, friction, or chemical drying caused by glove use, particularly with powdered gloves.
Exposure Sources
The most clinically significant exposures to natural rubber latex occur in medical and dental settings through gloves, catheters, dental dams, blood pressure cuffs, and surgical equipment. Outside healthcare, common sources include balloons, rubber bands, elastic in clothing, some adhesives, carpet backing, and rubber toys. Not all products labeled "latex" contain natural rubber — many contain synthetic materials. Patients with confirmed natural rubber latex allergy should check product labels and, when uncertain, contact manufacturers directly.
Airborne latex exposure is possible when powdered latex gloves are used — the powder can carry latex proteins into the air and reach the respiratory tract. Powder-free gloves substantially reduce this exposure pathway, and many healthcare facilities have moved away from powdered latex gloves as a result.
Latex-Fruit Syndrome
Some proteins in natural rubber latex are structurally similar to proteins in certain foods. People with latex allergy may develop reactions to these foods — a phenomenon called latex-fruit syndrome or latex-food syndrome. Banana, avocado, kiwi, and chestnut are the most commonly reported cross-reactive foods. Other foods, including papaya, peach, tomato, and potato, have been reported in individual cases. The degree of cross-reactivity varies among individuals. Not everyone with latex allergy develops food reactions, and food cross-reactions can range from mild oral itching to, rarely, anaphylaxis. Report any unexpected food reactions to your allergist for evaluation.
Diagnosis and Avoidance
Clinical evaluation includes a detailed history of reactions in relation to latex exposures — glove use, medical or dental procedures, balloon contact, or food reactions. Latex-specific IgE blood testing is the standard diagnostic tool in the United States; skin-prick testing is also used but should only be performed in a setting prepared to manage a potential allergic reaction. A positive test result requires interpretation in the context of clinical history. Avoidance of natural rubber latex products and clear communication with all healthcare providers are the primary management strategies. Wearing medical alert identification and carrying prescribed epinephrine for those at anaphylaxis risk are appropriate.
School and Workplace Planning
Children with latex allergy should have documentation on file with their school, specifying the allergy and including guidance on common latex sources in school environments such as art supplies, science equipment, and balloons at school events. Healthcare workers who develop latex allergy face occupational challenges that may require workplace accommodation, use of synthetic-glove alternatives, or, in some cases, role changes. Discussion with an occupational-health specialist is appropriate for workers with occupational latex sensitization.
