A separate total IgE measurement (measuring all IgE antibodies regardless of allergen specificity) may also be ordered in certain clinical contexts, though it has limited diagnostic utility on its own for identifying specific allergens.
Advantages Over Skin Prick Testing
Blood testing has specific practical advantages that make it the preferred or necessary approach in certain situations:
- Antihistamines do not interfere: IgE antibody concentrations in the blood are not affected by antihistamine medications. Patients who cannot safely stop antihistamines — because of severe ongoing urticaria or other reasons — can have blood testing without holding their medication.
- Skin condition is not a barrier: Patients with widespread active eczema, psoriasis, or dermatographism (where the skin reacts to pressure alone) may not have sufficient unaffected skin to read skin test results reliably. Blood testing does not require normal skin.
- No systemic reaction risk from the test: The blood draw itself carries no risk of triggering an allergic reaction. This is relevant for patients with a recent history of very severe anaphylaxis where repeat allergen exposure through skin testing might theoretically carry some risk.
- Centralized laboratory reading: Results are generated in a standardized laboratory, which eliminates variability related to the technique and skill of applying and reading skin tests.
Limitations Compared to Skin Prick Testing
Blood testing also has disadvantages relative to skin testing:
- Results are not immediate: Laboratory analysis takes days to return, unlike skin tests which provide results within a single office visit.
- Sensitivity varies by allergen: For some allergens, skin prick testing may be somewhat more sensitive than blood testing. The clinical significance of this varies by allergen and patient population.
- Cost: Blood tests for multiple allergens are generally more expensive than skin prick testing panels.
How to Interpret Results — Sensitization vs. Clinical Allergy
A positive blood test result (specific IgE above the reference range) indicates that the immune system has produced IgE antibodies to that allergen — the patient is sensitized. This is not the same as having a clinical allergy. Many sensitized individuals never develop symptoms when exposed to that allergen in real life. Conversely, the absolute level of specific IgE does not reliably predict how severe a reaction would be. Result interpretation requires correlating the test level with:
- The patient's clinical history — what symptoms occur, under what circumstances, and in temporal relationship to what exposures
- The clinical context — age, other conditions, diet, and living situation
- In some cases, an oral food challenge under medical supervision to confirm or rule out clinical food allergy

A Caution About Broad Food Allergy Panels
Ordering large untargeted food allergy panels — testing 50 or more foods at once without specific clinical indications — is discouraged by AAAAI, ACAAI, and other professional allergy organizations. These panels frequently produce positive results for foods a patient eats regularly without any reaction, because sensitization is common and does not equal clinical allergy. Reacting to these results by eliminating multiple foods can cause unnecessary dietary restriction, nutritional problems, and significant anxiety, particularly in children. Blood testing for food allergy is most clinically meaningful when ordered for specific foods based on the patient's history of reactions.
Component-Resolved Diagnostics
Beyond whole-allergen extract testing, component-resolved diagnostics (CRD) tests individual proteins within an allergen rather than the whole extract. For example, testing to specific peanut proteins (Ara h 2, Ara h 8, etc.) can provide information about whether a positive peanut test reflects sensitivity to the more clinically significant heat-stable storage proteins versus the cross-reactive proteins that are associated with pollen-food allergy syndrome and generally milder oral reactions. CRD is available for several important allergens and can help refine risk assessment and management decisions. It is ordered by an allergist based on specific clinical questions.
