Before Your First Appointment
The diagnostic process benefits significantly from preparation. Before seeing a clinician about suspected allergy, tracking your symptoms systematically makes your history far more useful. Note when symptoms occur, how long they last, what you were doing or eating beforehand, where you were, whether symptoms are seasonal or year-round, and what, if anything, relieves them. Photographing visible reactions such as hives or contact dermatitis at the time they appear provides documentation a clinician cannot obtain after the fact. Primary Care or Allergist?
The appropriate starting point depends on the complexity and severity of your suspected allergy. A primary care physician can evaluate mild or straightforward presentations, prescribe common allergy medications, and refer you if the picture is unclear or symptoms are not responding. A board-certified allergist is the appropriate specialist for complex or uncertain presentations, for suspected anaphylaxis risk, for anyone considering immunotherapy, and for situations where the specific allergen has not been identified. In practice, many people first raise allergy concerns with their primary care provider and are referred to an allergist when initial management is inadequate or more detailed diagnostic workup is needed.
The First Allergist Appointment
An initial allergist visit typically begins with a detailed clinical history. The allergist will ask about the nature and timing of your symptoms, the circumstances in which they occur, prior reactions and their severity, family history of atopic conditions, current medications, and whether any interventions have helped or worsened symptoms. This history is the foundation of the diagnostic assessment — testing is used to confirm or clarify a hypothesis formed from the clinical picture, not as a fishing expedition performed without a prior framework. Coming with organized notes from your symptom tracking and a list of current medications significantly improves the quality and efficiency of this initial visit.
Choosing Between Skin and Blood Tests
Not every suspected allergy requires the same test, and not every test is appropriate for every situation. The choice between skin testing, blood testing, and other procedures depends on the allergen suspected, the patient's current medications, existing skin conditions, and the degree of reaction risk. Antihistamines must typically be withheld for 3 to 7 days before skin testing, depending on the specific medication, because they suppress the histamine release that produces a positive result. Other medications including tricyclic antidepressants and some heartburn medications can also suppress skin test responses. The allergist's office will provide specific instructions about which medications to stop and for how long before your appointment.
What Happens During Skin Testing
Skin prick testing is the most commonly performed initial allergy test. Small amounts of specific allergen extracts are applied to the forearm or back, and a lancet lightly pricks the skin through each droplet. After approximately 15 to 20 minutes, any raised wheal — a hive-like bump — at an allergen site indicates a positive result. The size of the wheal provides some indication of the degree of sensitization, but does not directly translate to clinical allergy severity. Controls are included: a histamine control (which should produce a wheal in anyone) and a saline control (which should not), confirming that the test is functioning correctly.
Skin prick testing is generally safe and well tolerated. Local itching and small hives at test sites are expected and resolve within an hour or two. Systemic reactions are rare but can occur; allergist offices are equipped to manage them. If your skin is affected by active eczema in the testing area, or if you have taken antihistamines recently, skin testing results may not be interpretable and blood testing may be preferred.
Blood (Specific IgE) Testing
Specific IgE blood tests — sometimes called RAST or ImmunoCAP tests — measure the amount of IgE antibody directed against specific allergens. Unlike skin tests, they are not affected by antihistamines and can be performed regardless of skin condition. Results are typically available within several days to a week. Specific IgE test results are reported quantitatively and classified by level. Higher values indicate greater sensitization, but sensitization level does not reliably predict clinical allergy severity or the degree of reaction to expect on exposure. Elevated specific IgE to a food in someone who has never reacted to it requires careful clinical interpretation, not automatic dietary elimination. Interpreting Sensitization in Context
A positive allergy test — whether skin or blood — establishes sensitization, not diagnosis. Many people are sensitized to allergens they tolerate without symptoms. Interpreting a positive result requires asking: Has this person had symptoms when exposed to this allergen? Are the symptoms consistent with an allergic mechanism? Does the degree of sensitization correlate with the clinical history? Without a supportive history, a positive test may reflect cross-reactive proteins or sensitization without clinical relevance. Conversely, a negative result in someone with a strong clinical history does not definitively rule out allergy. Some allergies — particularly non-IgE-mediated reactions — do not produce positive results on standard IgE-based tests.
Oral Food Challenges
An oral food challenge (OFC) is the definitive procedure for confirming or ruling out a food allergy diagnosis. Under controlled conditions in a medical setting with emergency equipment and trained personnel present, the patient consumes gradually increasing amounts of the suspected food while being observed for signs of a reaction. If no reaction occurs up to a diagnostic dose, clinical food allergy is considered ruled out. If a reaction occurs, it is treated immediately and the diagnosis is confirmed. When Multiple Visits Are Needed and Follow-Up Planning
Some diagnoses require more than one visit. Complex presentations with many possible allergens, unclear histories, or the need to wait for antihistamine washout before skin testing are common reasons. Follow-up visits allow for result review, treatment plan adjustment, and — where immunotherapy is appropriate — initiation and monitoring of the treatment course. After a confirmed diagnosis, the follow-up plan should address not only avoidance but also emergency preparedness. Anyone with a diagnosis that carries anaphylaxis risk should leave the allergist's office with a prescription for epinephrine, a written emergency action plan, and clear guidance on when and how to use the device. The diagnostic timeline transitions into active management only when these pieces are in place.
